Position Description

This position's duties will fall primarily under the Facility Outreach and Community Integration Services (FOCIS)/Money Follows the Person (MFP) unit located at the Las Vegas Medicaid District Office. FOCIS is a program created by the State of Nevada in response to the Federal Olmstead Decision. This mandate requires that individuals residing in nursing facilities, hospitals, or rehabilitation centers be given the opportunity to explore resources as an alternative to living within an institution. This position will assist walk-ins in the District Office lobbies to provide the recipient information regarding benefits and to troubleshoot complicated access to care issues. Health Care Coordinators (HCC’s) evaluate individuals referred to the medical services program and provide ongoing case management services to Medicaid eligible clients; evaluate the need for medical services, treatment, equipment and supplies and authorize payment; screen individuals to determine appropriate level of care in nursing homes; review programs and participate in program development. Health Care Coordinators in this position use multiple databases to research recipient data, eligibility information, and computer system interface issues.

Minimum Qualifications

  • Licensure or provisional licensure as a Social Worker or professional licensure or certification in a medical specialty applicable to the assignment.

Special Requirements

  • A valid driver's license or evidence of equivalent mobility is required at the time of appointment and as a condition of continued employment.

  • Professional Licensure or certification as a social worker, nurse on in a medical specialty issued by the appropriate licensing board in the State of Nevada at the time of appointment. Incumbents must maintain professional licensure or certification in their specialty as a condition of continuing employment.
  • This position is required to travel up to 50% of the time
