Description: This position can be part time or full time depending on the candidates preference, 20 to 40 hours per week.
SUMMARY:The Case Manager is primarily responsible for providing services to clients who are HIV positive. The Case Manager will coordinate services for Ryan White eligible clients. The position requires at least 20 hours per week but can be full time up to 40 hours per week and will include some travel. The Case Manager will work closely with clients living with HIV/AIDS who may have psychosocial and/or health related needs. Case Management services is a strength-based approach to service that includes treatment adherence, counseling, coordination and follow-up of medical treatments, client advocacy, and assistance in obtaining housing, financial support, legal services, social support, and other needed services. The goals of Case Management are to ensure that clients with HIV/AIDS have timely access to comprehensive medical care and social services, prevent disease transmission, minimize HIV progression, and to promote and support client independence and self-sufficiency. Starting wage of $17.00 DOE Requirements:

MINIMUM QUALIFICATIONS:

  • A bachelor’s degree in a health or human services field is preferred; or
  • Extensive experience defined as specialized case management training; or
  • Associate’s degree and one to two years’ experience in a human services related field such as: social work, psychology, or nursing.
  • For social work degree must provide current Idaho State license.
  • Must pass state background check.
  • Must possess a valid Driver’s License and current insurance.
  • Must have own transportation.

  • Excellent written and verbal communication skills.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

DUTIES AND RESPONSIBILITIES:

  • Conduct face to face HIV medical case management assessments to identify factors that impact patients’ ability to adhere to their medical plan.
  • Develop individual service plans based on assessments and coordinate the development of the services plan with the client’s participation.
  • Coordinate service delivery to ensure continuity of care.
  • Maintain frequent contact with providers and clients to ensure that services are provided as indicated in the service plan.
  • Provide documentation, complete annual and bi-annual IDHW recertification’s and complete data entry into CAREWare and the EMR as required to maintain patient records.
  • Assist patients with accessing resources (medication, finances, housing, etc.)
  • Counsels eligible patients regarding available benefits and entitlements; facilitates referrals to FHS, private and public resources or programs such as Medicaid, Medicare, State Pharmacy Assistance Programs, Pharmaceutical Manufacturers Patient Assistance Programs and other State or local health care and support services.
  • Assists patients with the Linkage to Care to help ensure that patients living with HIV/AIDS who have moved to Idaho, are connected with a care provider.
  • Other assigned job functions in conjunction with Idaho Health and Welfare Part B guidance.

  • Completes assigned continued education and mandatory compliance training as assigned.
  • Participates in Safety, Security and Emergency Preparedness drills and training as assigned.
  • Attends scheduled team and interdepartmental meetings as directed. Participate in conference calls and training in and out of the state as requested.
  • Work with Supervisor Case Manger on administrative reporting.
  • Prepare and submit monthly billing statements.
  • Actively manage phones and emails.
  • Perform other duties as assigned.
