New Job Intensive Case Management Lmsw/Lpc In Louisiana

Title of Position: Intensive Case Management LMSW/LPC

Description of Duties and Responsibilities:

• Conduct screening and assessment of high-risk needs, such as substance use, psychiatric symptoms or hospitalization, harm to self/others, abuse/neglect, or domestic violence.
• Provide case management services, including comprehensive care management, care coordination and healthy promotion, comprehensive transitional care, enrollee and family support, referral to community and social supports, use of health information technology to link services
• Provide case management services in a person/family-centered, strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner
• Plan and coordinate case management services, including referral and escort to medical, psychiatric, substance use, crisis intervention, legal, housing, education/employment, entitlements, social, and peer support services
• Maintain engagement with individuals who are often disengaged from care, have difficulty adhering to treatment recommendations, or have a history of homelessness or involvement in the criminal justice system
• Conduct monitoring and follow-up services to promote continued engagement in referred rehabilitative and treatment services.
• Participate in quality improvement activities.

Minimum Qualifications

• Master’s degree in Human Services or related field - Required
• 2 years’ experience with MH/SUD population - Preferred

Supervisory Relationships:

• The Intensive Case Management LMSW reports to the Intensive Case Management Supervisor/LPC – License Professional Counselor

Personal Qualities:

• Good verbal and written communication skills necessary
• Ability to work as a team member and as a leader

Amount of Travel/Other Special Conditions or Requirements: N/A

Salary Range: $45,000 – $70,000

Hours Per Day or Week: 40/week

Job Type: Contract

Pay: Up to $70,000.00 per year

Schedule:

• 8 hour shift

Ability to commute/relocate:

• New Orleans, LA: Reliably commute or planning to relocate before starting work (Required)

Education:

• Master's (Preferred)

Experience:

• Case management: 2 years (Preferred)

Willingness to travel:

• 25% (Preferred)
