Position Summary
This position is funded by a grant for a duration of time as defined by the grantor.
Works under the general supervision to provide assistance to Tuberculosis (TB) patients to ensure assigned clients have resources and assistance necessary to comply with control measures by serving as a liaison with hospitals, health care providers, housing assistance vendors, and social service agencies.
Pay Grade: 48
Salary Commensurate with Experience and Education
Minimum Qualifications
100.00% - 1. Two (2) years of experience in professional social work; and
2. Bachelor's degree from an accredited college or university or the National Council on Social Work Education in social work.
3. Licensed as a Social Worker.
4. Experience working with Tuberculosis patients preferred.
5. PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED Duties and Responsibilities
1. Visits and networks with clients or agencies to obtain necessary information to provide assistance to clients.
2. Makes home visits to investigate and validate clients" needs.
3. Confers with public and private agencies to communicate the TB Outreach program and client needs and make necessary arrangements.
4. Attends healthcare fairs as required and recommended.
5. Refers patients and their families to appropriate agencies for assistance.
6. Evaluates current needs of patients prior to alignment with proper resources.
7. Works closely with the assigned case managers in securing and recommending proper resources.
8. Solicits hotel accommodations and patient housing for TB patients in isolation.
9. Completes required casework and reports.
10. Assists or initiates special studies or surveys.
11. Presents and educates the community of Tuberculosis (TB) educational programs.
12. Performs other related duties as required or directed. KSA's
KSAs - 1. Excellent written and oral communication skills.
2. Knowledge of the principles and techniques of social work.
3. Ability to maintain professionalism with people of all economic and social levels.
4. Ability to use good judgment in evaluating situations and making decisions.
5. Ability to work express ideas clearly and concisely.
6. Proficient in the use of MS Office ® (Excel, Word, and PowerPoint) or similar software. Physical Requirements
1. The physical activities of this position may include walking, reaching, kneeling, ascending/descending stairs, stooping, and repetitive motion.
2. Requires lifting and transporting materials and supplies up to 20 lbs. Special Requirements
1. Must possess a valid Tennessee driver license or secure one as of employment date.
2. Must have daily access to a personal operable automobile and meet county requirements for automobile insurance upon date of employment.
3. Work will require attendance at meetings and activities on weekends and/or after regular work hours.
4. Must be able to travel to locations outside of the office and County as required.
5. Must be licensed as a social worker.
6. Must maintain license throughout duration of employment. Disclaimer
