Providence is calling a Lifestyle Coach (Training and Development, Variable, 0.0 FTE) to Providence St. Mary Medical Center in Walla Walla, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.

In this position you will:

  • Consistently demonstrate adherence to the principles and requirements outlined in the Commitments and Compliance Integrity Program and any other organization or departmental materials related to compliance with laws, regulatory requirements.
  • Consistently demonstrate adherence to Medicare and Medicaid regulations which directly or indirectly relate to the performance and job expectations.
  • Employee demonstrate knowledge of PSMMC’s policy on Patients Rights and Responsibilities and respects these rights and responsibilities in carrying-out daily activities.
  • Actively participate in departmental and organization performance improvement activities in accordance with hospital and joint commission standards.
  • Demonstrate leadership by performing his/her work in a manner that meets or exceeds the level of care and service that PSMMC promotes to the communities we serve.
  • Participate in hospital and department specific practices to maintain a clean, safe and comfortable environment for patients and staff.
  • Maintain the security and confidentiality of data and information used in performance of his/her daily duties conforming to hospital policy, laws and regulations as appropriate.
  • Follow established policies and procedures to identify and reduce the risks of acquiring or transmitting infection.
  • Maintain positive working relationships and fosters a cooperative work environment.
  • Communicate effectively with patients, physicians and staff in a professional manner.
  • Provide clear, concise, pertinent reports/information.
  • Facilitate sessions over the course of the 1-year program (weekly sessions for 16-26 weeks followed by monthly sessions for the remainder of the year).
  • Create an environment that is motivating, supportive, non-competitive, and non-judgmental.
  • Continually reinforce and integrate session material, build on concepts by referring back to previous sessions and discussions.
  • Promote group discussion whenever possible and appropriate.
  • Engage group dynamics to build accountability among participants.
  • Support participants by providing information, encouraging progress, and working with the group to address challenges or barriers that arise.
  • Encourage goal setting on a weekly basis.
  • Make learning a shared responsibility for the group.
  • Prepare for each session (i.e. review lesson plan and content for each session, provide feedback for participants on their food and activity trackers).
  • Deliver the National Diabetes Prevention Program lifestyle change program as it was designed. Lifestyle Coaches can add their personal strengths as a coach and provide culturally relevant examples of food, types of physical activity, barriers, etc. but should not alter the key messages of the curriculum.
  • Maintain high standards and expectations of self and participants including attendance, starting on time, weekly weigh-ins, self-monitoring, and problem solving.
  • Be accessible to participants before and after sessions to answer questions and follow-up on any questions that cannot be answered during class time.
  • Follow-up with participants outside of class if they are unable to attend a class.
  • Record weekly data for each participant for submission to the Diabetes Prevention Recognition Program (DPRP).
  • Follow all DPP documentation and data reporting workflows and seek advice of the Program Coordinator as needed.
  • JOB SPECIFIC FUNCTIONS:
  • Demonstrate self-motivation by independently following through with assigned committee or project work.
  • Demonstrate knowledge of State, Federal, and the joint commission standards pertinent to scope of practice within the clinical operations of the hospital supporting all members of the team to achieve compliance.
  • Serve as a clinical resource for staff, students, and community groups as needed.
  • Demonstrate knowledge and accuracy using hospital computer information systems as determined by department access restrictions.
  • HUMAN RESOURCES:
  • Employee complete all tasks and duties accurately and timely within assigned shift. Demonstrates initiative to perform job duties autonomously.
  • Attend mandatory and other education offerings per department and hospital requirements. May assist with education on the unit.

Qualifications:

Required qualifications for this position include:

  • H.S. Diploma or GED

Preferred qualifications for this position include:

  • Health coach certification from ACE (American College of Exercise)
  • National Consortium for Credentialing of Health and Wellness Coaches
  • National Society of Health Coaches or another recognized credentialing agency
  • One (1) year leading group behavior change sessions.
