Summary

The incumbent serves as the Patient Centered Management Module (PCMM) Facility Coordinator for all Primary Care and Mental Health Services at the Sioux Falls, SD VA Medical Center. The position has the primary responsibility for management of PCMM software programs, performs management, and workload and program analyses. The position is located within and reports to the Director of Clinical Support.


Responsibilities

The position provides high-level operational oversight and management expertise of the PCMM Program and has responsibility to engage in developing, analyzing, evaluating and advising on/or improving the effectiveness of methods and procedures.

Duties include, but are not limited to:

  • Assist operation officials in implementing identified improvement actions and plans through participation in organization improvement work
  • Complete comparative analysis of VSSC reports or data sets with local PCMM date to identify data variance and ensure date integrity
  • Delegate PCMM Web user permissions for facility staff members
  • Ensure the PCMM users are knowledgeable in PCMM functionality and utilization
  • Establish and manage PCMM Web position set up; staff and FTE assignment, as well as patient assignments for all primary care and mental health locations
  • Ensure that PCDPC data is entered for each PACT team member in PCMM and entered as a portion of an FTE employee is kept current and accurate
  • Maintain oversight of the Multi-PAC approval disposition to ensure timely completion of active assignments
  • Meet regularly with facility leadership and group practice manager to review provider panel management data, including access for face-to-face and telephone care
  • Participate in monthly national calls with the national PCMM Coordinator to ensure alignment with national policy, receive updates and share strong practices
  • Participate in PCMM training activities and in national and/or VISN PCMM conference calls and discussion as needed
  • Serve as the facilities technical expert for PCMM Web with the ability to troubleshoot and resolve user issues
  • Serve on facility committees as needed, as the PCMM subject matter expert
  • Other duties as assigned
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.

    Qualifications

    To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 04/12/2021.

    Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.

    To meet the minimum qualifications for this position at the GS-07 grade level, you must meet one of the following:
    • Specialized Experience: One (1) full year of specialized experience equivalent to the GS-05 level in the Federal service that equipped you with the particular knowledge, skills, and abilities to perform successfully the duties of the position, and that is typically in or related to the work of this position. Examples of specialized experience would typically include, but are not limited to: utilize creativity and judgment in selecting methods and approaches for tasks; resolve conflict and determine potentially controversial issues to present to supervisor level; operational oversight and management expertise of programs; develop, analyze and advise improvement methods and procedures; interpretation of policy and guidelines to support the program; understanding of how the primary care systems work. NOTE: Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment.
    OR,
    • Education: Have successfully completed 1 full year of graduate level education or superior academic achievement that demonstrates the knowledge, skills, and abilities necessary to do the work of this position.

    OR,
    • Combination: Possess equivalent combinations of successfully completed graduate level education (in excess of the first year or 18 semester hours) and specialized experience to meet total experience requirements.     To meet the minimum qualifications for this position at the GS-09 grade level, you must meet one of the following:
    • Specialized Experience: One (1) full year of specialized experience equivalent to the GS-07 level in the Federal service that equipped you with the particular knowledge, skills, and abilities to perform successfully the duties of the position, and that is typically in or related to the work of this position. Examples of specialized experience would typically include, but are not limited to: apply a wide range of methods used to gather, analyze, and evaluate information concerning management processes; facilitate systems redesign activities, including cycle time studies and process flow mapping; possess analytical skills, problem-solving methods and techniques; understand operational problems and issues and develop a hypothesis and solutions; improvement of program effectiveness and organizational productivity; develop new or modified work methods, organizational structures, records and management processes and administrative procedures; understanding of standardized clinic operational practices and procedures to conduct studies of clinic work processes. NOTE: Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment.
