New job Hospital Liaison in New Hampshire | 2021

The Hospital Liaison ensures continuity of care through effective discharge planning and care coordination of patients admitted to a psychiatric hospital from Lakes Region Mental Health Center (LRMHC) and all patients being discharged from a psychiatric hospital to the Region 3 catchment area and LRMHC services. It is the responsibility of the Hospital Liaison to facilitate and organize provision of services within their assigned area and to enhance the awareness of mental health issues for patients and their families through consultation and facilitation of educational experiences to staff, other service providers, schools, and the community at large.

Essential Responsibilities

• Coordinates discharge of patients to include community reintegration and access to clinically appropriate level of care
• Provides timely updates on admissions and discharge planning to the treatment team
• Meets with patients in the hospital, hospital staff and patient’s families as appropriate
• Evaluates barriers to discharge and works with patient, hospital and LRMHC to remove barriers Assessments, psychotherapeutic interventions, case coordination, crisis intervention.
• Prepare and maintain required clinical and administrative documentation.

Qualifications
Master’s degree in Social Work, Psychology, Counseling, Human Services or other related field required. 2 to 3 years experience working as a Master’s level clinician preferred.
