Community Liaison Responsibilities:

· Initial meeting with potential new clients to develop a care plan identifying all expectations of care by the client and their family

· Identify and facilitate opportunities for education, seminars, presentations, speaking engagements, events and any other format that will provide as well as be a resource for our clients, family members and circle of support

· Conduct “supervisory visits” with current clients for quality assurance or care being provided

· Engage with existing caregivers regarding their individual needs to deliver the consistent message that they are a highly valued member of the team

· Follow all policies and procedures, including those in the Administrative Employee Caregiver Handbook and Staffing Guidelines

Community Liaison Requirements:

· Proven work experience in building positive, lasting relationships and growing private pay business

· Case Management experience a plus

Job Type: Full-time

Pay: $48,000.00 - $50,000.00 per year
