Patient Engagement Specialist

As a Patient Engagement Specialist, you will have the opportunity to improve the quality of life and quality of healthcare for Medicare beneficiaries with or at risk for chronic kidney failure and end-stage renal disease. You will provide technical assistance to support the achievement of Quality Improvement goals. You will also develop and implement patient and family engagement tactics with the ESRD team.
This position is contingent upon award.

What You'll Do:
  • You will develop and implement tools to promote patient and family engagement with the ESRD team.
  • You will facilitate patient and family engagement events across the ESRD community.
  • You will serve as convener, organizer, motivator, and change agent, leveraging technology to provide outreach and education.
  • You will serve as partners in quality improvement with patients, practitioners, healthcare providers, other healthcare organizations, and other stakeholders.
  • You will secure commitments to create collaborative relationships with other stakeholders and partners.
  • You will achieve and measure changes at the patient level though data collection, analysis, and monitoring for improvement, and based on the outcome of the analysis, you will determine areas for improvement.
  • You will disseminate and spread best practices, including those relating to clinical care, quality improvement techniques, and data collection through information exchange.
  • You will participate in the development of a CMS national framework for providing emergency preparedness services for the ESRD community.

What You'll Need:
  • Patient and Family engagement skills
  • Experience as a licensed clinical professional, community health professional, public health professional, community development professional, or social work professional.
  • 3 - 5 years experience working in direct collaboration with providers, community organizing, population health or public health and/or health care quality improvement in specific areas as needed to be designated by the contract
  • Experience working with the ESRD community
  • Human-centered design skills
  • Experience with quality improvement initiatives
  • Functional PC knowledge
