JOB DESCRIPTION SUMMARY
General Statement of Duties
The Investigator will contact those diagnosed with COVID-19 to elicit contacts during infectious period, provide education and instructions for safe self-isolating/ quarantine. This position will conduct or assist with workplace and healthcare facility notification and conduct outbreak/cluster investigations. This position will lead or assist in the development of materials to support investigation activities including contact tracing. Investigators are required to follow all policies and procedures provided by Lenawee County Health Department (LCHD), and comply with LCHD training regarding confidential information related to personal information.

Supervision Received
Work is performed under the general supervision of the Director of Personal Health, Deputy Director of Personal Health, Communicable Disease RN, or designee.

Primary Duties and Responsibilities

Conduct case investigation for confirmed and probable cases with elicitation of contacts in timeframe specified by Michigan Department of Health and Human Services.
Provide education and resources regarding safe self-isolation/quarantine practices in the home setting.
Conduct follow-up phone calls with cases to determine when self-isolation/quarantine may be safely discontinued.
Conduct confidential employer notification if workplace risk was present.
Conduct confidential notification to healthcare facilities if risk was present.
Provide guidance to healthcare providers, community partners, and facilities.
Create and maintain disease investigation protocols and other tools or resources (e.g. forms, scripts).
Conduct contact tracing and/or monitoring activities.
Data entry
Maintain daily contact with supervisor.
Other duties as assigned.

Effectively interpret guidance from Michigan Department and Human Services and Center for Disease Control and Prevention.
Excellent interpersonal skills required to engage with a culturally diverse population.
Ability to show empathy to individuals under stress and in crisis.
Excellent organizational and communication skills.
Ability to handle confidential information with discretion and professionalism.
Proficiency with computers, electronic databases and spreadsheets.
Familiarity with Lenawee County geography; cities/towns/locations required.
Ability to speak, read, and write English; ability to speak, read and write Spanish is desired, but not required.

Qualifications

Bachelor's degree in Health Sciences, Public Health, or related field, or equivalent experience OR;
Degree in health field (LPN, RN; Health Sciences or similar field) or equivalent experience.

Working Conditions

This is a part-time, temporary and limited to less than 1,000 working hours per calendar year.
Requires a high degree of schedule flexibility to respond to changes in caseload and departmental emergency situation during non-routine work hours.
Weekend and evening work will be required.
Work may be required to be performed both in-person and remotely.

Physical Demands and Work Environment

Duties require sufficient mobility to work in a normal office setting and use standard office equipment including a computer, vision to read printed materials and a computer screen and hearing and speech sufficient to communicate in person or over the telephone.
Duties require sufficient mobility to work in a field environment including restaurants, businesses, residences.
Should be able to bend, stretch, reach, and lift up to 25 pounds.
