Wraparound Facilitator/Care Coordinator, Manchester, NH. The Care Coordinator will meet with families, facilitate wraparound meetings, and perform intensive care coordination; coordinating referrals to other supports and services as well as developing crisis plans and a family’s plan for care. In the wraparound process, the family will drive the work and the Coordinator will assist them in identifying underlying needs and developing tools and strategies to enhance the self-efficacy of the family. Coordinators must complete the NH Wraparound Coordinator certification process through completing a three-day facilitator training and ongoing, intensive coaching sessions. Additional responsibilities include maintaining a client caseload, attending weekly planning and team meetings and monitoring services to best meet the client and family needs. A Bachelor's degree in Social Work or related field with a minimum of 3 years’ experience working with young children and families in a social service capacity required. Other skills necessary are the ability to facilitate large group meetings with various families and partners, complete paperwork timely and effectively and work within a multidisciplinary team.
